To be a member of one of our plans:

  • You must live in our service area (coverage area),
  • be entitled to Medicare Part A,
  • be enrolled in Medicare Part B

If you are enrolling in SecureCare SNP (HMO D-SNP), you must also have Medicaid through your state.

If you currently pay a premium for Medicare Part A and/or Medicare Part B, you must continue paying your premium in order to keep your Medicare Part A and/or Medicare Part B and remain a member of this plan.  This is true even if the plan premium is $0.

The Health Plan's contract with CMS is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed.


How do I enroll?

After completing the online application, you will receive a confirmation number. This number verifies that you have chosen to enroll and Medicare will notify The Health Plan of your enrollment.

Medicare beneficiaries may also enroll in a SecureCare (HMO), SecureCare SNP (HMO D-SNP) or SecureChoice (PPO) plan through the CMS Medicare Online Enrollment Center located at medicare.gov.

Changing your method of premium payment: If you decide to switch to premium withhold or move from premium withhold to direct bill, it could take up to three months for it to take effect and you will be ultimately held responsible for those premiums.


How do I terminate membership?
